Return to Play Phases 5 & 6 Criteria for Athletes

1) Must be symptom free for a full work/school day and off medications

2) No longer using academic accommodations during a full school day

3) Demonstrates normal performace on academic/mental and physical daily tasks

4) Cognitive testing has returned to baseline/normal averages

5) Re-evaluation by healthcare team for release to progress to full contact activities

*If not met, continue to phase 3 & 4 until criteria are met. Let your healthcare provider know if you are having difficulties completing phases

Return to full contact practice:

Individual is cleared to progress to contact activity by his/her healthcare team, he/she can participate in this phase in a team practice environment

  • Participation in the full duration of a normal practice with rest periods as tolerated

  • Must maintain symptom-free status throughout and after team practice in order to advance to a regular game situation

Did you participate in a full contact practice session with your team without return of concussion symptoms?

No? recover for 24 hours and return to phase 4, progress as tolerated

*Follow up with your healthcare team if repeated attempts at progression are unsuccessful

Yes? contact healthcare team for release to full participation and continue to Phase 6
